Need help with college resume, for high school student?

i'm a high school student applying to a college, and i was told to make a college resume to give to my teachers for a college recommendation. I've never made a resume before, and i tried looking up templates online and stuff, and got confused because there are so many different things.

I know what you are talking about, I've Just never heard it called "college resume"

Resumes are usually used when seeking employment.
Curriculum Vitaes are usually used in regards to academic acheivements.

What you want is technically neither. But I made something like this when I was getting ready to ask teachers for recommendations. So here's what you should include:

First Page: You name, address, phone, email... any way you can think of that the teacher who is providing the recommendation to get a hold of you.
-Also include a breif paragraph about yourself. Why do you want to college, why "those" colleges (you chose), What do you want to study, and why? What career do you hope to obtain after school? How much schooling are you going to get... etc.

Second Page: Provide you test scores (PSAT/SAT/ACT), and maybe your precentile ranking

Third Page: Attach a transcript of your high school classes with grades.

Fourth Page: List your extracurricular activities... clubs or organizations you've joined and participated in, and whether you held a leadership position (president, VP...etc)
-Sports you play/played, position you played
-Voluteered activities
-Interships you did

Also include any academic acheievements (Awards, Scholarships, Honors, etc(

Fifth Page: Names and address of colleges you are applying to.
Include what program you applying to, and maybe the school website (or program website), so you teacher can look up each.

You can also provide already addressed and stamped envelopes for your teachers who are providing you recommendations Anything to make their job easier means a better letter for you.

Hope that covers it. If your unsure as what you should put in this "packet" so to speak, as a teacher who knows you well, and they'll be more likely to help you.
